What we do
What is Enhanced Case Management?
Enhanced Care Management (ECM) provides comprehensive, whole-person care coordination for individuals with complex physical, behavioral, and social health needs. Working closely with Inland Empire Health Plan (IEHP) and local providers, our dedicated care team coordinates your health, behavioral care, and social resources so you have continuous support every step of the way.
We connect you with primary care doctors, behavioral health specialists, mental health therapy, psychiatry, and oral health providers to ensure seamless, continuous care.
Our team assists you with understanding and navigating health plans (IEHP / Medi-Cal) and keeping the essential benefits you are eligible for.
We assist with scheduling medical visits, arranging transportation to appointments, medication management, and post-discharge follow-up care after hospital or crisis stays.
We bridge the gap to essential social services including housing navigation, emergency shelter referrals, food resources, and local community assistance programs.

How to get started
Enhanced Care Management is available at no cost for eligible Inland Empire Health Plan (IEHP) members, including individuals experiencing homelessness with complex needs, frequent hospital or ER visits, serious mental illness or substance use challenges, youth with complex health needs, or those transitioning from care facilities or incarceration. For non-IEHP individuals, our sliding-scale charity care program ensures no one is turned away for inability to pay.
